During a joint exercise in the Indian Ocean in 1983, the British Royal Navy frigate HMS Ambuscade (Type 21) was observed to have sustained damage after a collision with the United States Navy destroyer USS Dale. The ships were conducting joint exercises. Dale was maintaining a steady course (14-18 knots), while Ambuscade was “leapfrogging” the destroyer, leading to the collision and occurred during a multinational operation involving complex coordination between fleets from various countries. The incident was then thoroughly reviewed to understand the factors that influenced the event.
